Integrated Primary Care
Primary Care services are now available at Macon County Public Health for Macon County residents
who do not have a primary care doctor and are between the ages of 21 and 64.
Please complete the Primary Care New Patient Medical Questionnaire and Release of
Information Form below and return them to Macon County Public Health at your earliest convenience.
A team of professionals will review the application to determine if MCPH can meet your needs. We may be
requesting additional information and/or your previous records.
You will be contacted on the status of your request. If you are approved, an appointment will be
scheduled. The first appointment will be a comprehensive examination to include a comprehensive
review of your health history and examination.
You will need to bring proof of household income
and/or insurance information to the first visit. If you have health insurance, it will be filed,
but you will be responsible for any co-payment and/or non-covered service charges.
If you do not
have health insurance, you may be eligible for discounted services which are based on household
income and family size. This will be discussed with you at your first visit.
