| Adult Protective Services: |
Investigation and treatment of referrals of abuse, neglect or exploitation of disabled adults age 18 and over. |
| Adult Guardianship: |
Legal custodian of adults who have been adjudicated incompetent by the Clerk of Superior Court. Provide case management. |
| "At Risk" Case Management: |
Assess, identify, and coordinate resources to meet the needs of clients who receive full coverage Medicaid and who need assistance with activities of daily living. |
Representative/
Protective Payee: |
Act as payee to clients who have been determined by the Social Security Administration as unable to manage their finances. |
| Adult Home Specialist: |
Monitor adult care home, family care homes, and facilities to ensure that state standards are met. Licensure and re-certification of new and existing facilities. |
| Adult Care Home Case Management: |
Provides case management services to Adult Care Home residents with special needs and receiving Special Assistance. |
| Adult Day Care Coordinator: |
Monitor, licensure, and re-certification of ADC Center's. |
| Medicaid Transportation: |
Assess eligibility and need for services including gas vouchers, reimbursements, and coordination of transit services. |
| In-Home Aid: |
Provide Level I Home Maintenance Services to eligible adults. |
| Adult Placement: |
Assist aging and disabled adult and their families with process of locating suitable nursing home or domicilary home placement. |
